Peptides and the New Science of Skin Longevity Image

September 20, 2026

I recently had the privilege of speaking on the Main Stage at RAADfest in Scottsdale about Skin Longevity—a subject I believe represents one of the most exciting shifts occurring in dermatology, aesthetics and longevity medicine.

For decades, our industry has talked about “anti-aging.”

But I believe we need to think bigger.

The goal should not simply be to make skin look younger. We should be asking:

Can we help skin function younger—and remain healthier, stronger and more resilient for longer?

That is the essence of Skin Longevity.

And emerging peptide science is becoming an intriguing part of that conversation.

We Need to Look Beyond the Wrinkle

One of the central messages of my RAADfest presentation was that what we recognize visually as “aging skin”—wrinkles, laxity, uneven pigmentation, thinning and changes in texture—is really the visible expression of much more complex biology occurring beneath the surface.

During my presentation, I organized this biology around several important Hallmarks of Skin Aging:

DNA Damage • Cellular Senescence • Extracellular Matrix Degradation • Mitochondrial Dysfunction • Declining Immune Surveillance • Barrier Dysfunction

These processes do not occur independently. They interact with one another over decades, influenced by both our intrinsic biology and extrinsic factors such as ultraviolet radiation and environmental exposures.

That changes the question we should be asking.

Instead of simply:

“How do I treat this wrinkle?”

I believe the better question is:

“What is happening biologically within this skin—and how can we help preserve its function?”

That is an entirely different approach to aesthetic dermatology.

Where Peptides Become Interesting

Peptides are short chains of amino acids, and some can function as biological signaling molecules—essentially delivering messages that influence cellular activity.

Peptides are certainly not new to skincare. What is changing is the sophistication with which researchers are investigating them.

Rather than looking only at whether a peptide-containing product improves the appearance of a wrinkle, researchers are increasingly examining how specific peptides may influence biological processes associated with aging—including extracellular-matrix remodeling, collagen signaling, oxidative stress, cellular senescence, mitochondrial function, proteostasis and autophagy.

That is an important evolution.

We are moving from thinking exclusively about cosmetic correction toward investigating biological preservation and regeneration.

Not All Peptides Are Created Equal

This is where I put on my dermatologist hat.

Seeing the word “peptide” on a skincare label does not automatically make a product effective—or a longevity intervention.

Peptides are highly specific molecules. Different sequences may deliver very different biological signals, and simply placing a peptide into a formulation does not guarantee that it will remain stable, penetrate to its intended target or produce a meaningful clinical effect.

So I want to know:

Which peptide? What pathway is it targeting? At what concentration? How is it being delivered? And what evidence demonstrates that it actually produces a meaningful change in human skin?

These distinctions are particularly important as “longevity” becomes increasingly popular in skincare marketing.

Laboratory and ex vivo studies can tell us a tremendous amount about how an ingredient might work. Human clinical trials are necessary to determine whether those mechanisms translate into visible and measurable benefits.

There is encouraging clinical evidence for some peptide approaches, but the evidence is not uniform across all peptides or formulations. Skin Longevity needs to remain science-driven—not trend-driven.

Mitochondria: The Energy Behind Healthy Skin

One area of Skin Longevity that particularly fascinates me is mitochondrial function.

Mitochondria produce the energy our cells require to perform virtually everything associated with healthy skin—repair, regeneration, protein synthesis, cellular communication and recovery from environmental stress.

As mitochondrial function declines with aging, our cells may become less efficient at performing those jobs.

At the same time, damaged proteins accumulate, autophagy and cellular recycling can become less efficient, oxidative stress increases, and senescent cells can accumulate.

This is why research examining peptides and other interventions that may influence these pathways is so interesting.

The future of skincare may increasingly involve identifying specific biological vulnerabilities associated with aging and targeting them more precisely.

That is a very different concept from simply finding the next “anti-aging” ingredient.

Skin May Be One of Our Most Accessible Biomarkers of Aging

This may be the part of Skin Longevity that excites me most.

Skin is unique among our organs.

We can see it. We can photograph it. We can measure it. We can treat it. And then we can follow how it responds over time.

At BelleSante™, we already use advanced imaging to objectively evaluate changes in pigmentation, vascularity, texture, pores, wrinkles and photodamage.

As the science evolves, I believe we will become increasingly sophisticated in correlating what we see externally with what is happening biologically underneath.

Skin may ultimately become much more than an aesthetic endpoint.

It may become one of our most visible and measurable windows into how we are aging.

The BelleSante™ Skin Longevity Playbook

PROTECT → REPAIR → REGENERATE → MEASURE → PERSONALIZE

PROTECT

Daily broad-spectrum sun protection and strategies to minimize environmental damage.

REPAIR

Evidence-based medical skincare—including appropriately selected retinoids, antioxidants and other targeted ingredients—to support healthier cellular function.

REGENERATE

When appropriate, use technologies and treatments that stimulate repair and remodeling, including injectables, lasers and light-based treatments, microneedling, ultrasound technologies, as well as regenerative and biostimulatory approaches.

MEASURE

Use advanced skin imaging and objective measurements whenever possible rather than relying solely on what we think we see in the mirror.

PERSONALIZE

Recognize that skin does not age independently from the person it belongs to. Genetics, hormones, metabolic health, cellular nutrition, muscle, inflammation, sleep and lifestyle all influence how we age.

The goal is not simply younger-looking skin.

The goal is healthier-functioning skin for longer.

Skincare Is Only One Part of Skin Longevity

This brings us to an important point.

No peptide, serum, laser or injectable exists in isolation.

Our skin is connected to the biology of the entire person.

Hormonal changes, metabolic health, nutrition, muscle mass, sleep, chronic inflammation, environmental exposures and our individual genetics can all influence how our skin ages.

That is why at BelleSante™, our approach increasingly connects what we do externally through the BelleSante Skin Gym™ with what we learn about the individual through our BioSculpt® Precision Wellness System™.

The future of aesthetic medicine cannot simply be about treating a face.

It must become increasingly preventative, measurable and personalized.

From Anti-Aging to Aging Healthier™

Peptides represent one exciting area within a much larger scientific evolution.

As research advances, I expect we will see increasingly sophisticated skincare strategies designed around specific biological pathways rather than generic claims of “anti-aging.”

But I don’t believe the future will be one miracle ingredient.

The future is understanding why we age, identifying which aspects of that biology we can meaningfully influence, measuring change whenever possible, and then combining the appropriate interventions for the individual sitting in front of us.

That was my message at RAADfest—and it is central to what we are building every day at BelleSante™.

We aren’t trying to stop aging.

We are learning how to Age Healthier™.

And when it comes to our skin, that means moving beyond simply treating what we see today toward preserving the health, resilience and function of our skin for the years ahead.

"I was having at a minimum of two migraines a week. My peripheral vision would be blacked out, nausea would set in, I would have a sharp and throbbing pain in my forehead and occipital area, as well as sensitivity to noise. After receiving Jeuveau injections in my masseters, forehead, and occipital areas from Dr. Romine, I went from having multiple migraines per week to one migraine in the last eight weeks. I am so appreciative to Dr. Romine for helping me feel normal again and allowing me to resume my normal schedule."
